Before Your Visit

In preparation for your appointment, we ask that you bring your insurance card or forms and a picture ID/drivers license with you. Please also bring a list of medications that you are currently taking, including dosage and frequency.

We suggest that you wear loose, comfortable clothing and sneakers or low heeled shoes for your visit. Please check our campus map for department locations within the Medical Center.
