Whether stored in a computer file, handwritten file or by other means, we are committed to keeping your information in a safe and secure environment that protects your privacy and confidentiality. Only the physicians and other health care professionals who are involved in your care will have access to your information. This will ensure that we provide the most appropriate and complete treatment for you.

You, your legal representative and anyone to whom you give written permission, will have the right to read or obtain a copy of the minimum necessary information from your medical record, in order to provide the assistance requested. Please note that your medical record is the physical property of the hospital and/or the physician practice.
