Case Management

The Case Management Department at Saints includes case managers and social workers who assist patients and their families in designing a discharge plan that meets their needs, as well as providing support during hospitalization. Your Case Manager will collaborate with you, your doctors and the health care team to assure the care you receive at Saints Medical Center focuses on your acute care needs. The Case Manager will assist you with identifying and planning for discharge beginning immediately after admission, keeping you aware of your clinical goals for discharge. The Case Manager will communicate with your insurance company as needed.

The Care Coordination Team consists of your Case Manager, Social Worker, your Physician and our Physician Advisor.  Your Social Worker is available for emotional support, completion of a Health Care Proxy, and assistance with community mental health and substance abuse resources.
