Pre-registration

Pre-registration is required before your visit. Patients may pre-register online by clicking here.  Alternately, patients may register by telephone. Please have your social security number and insurance card(s) ready when registering. If you do not register online or by telephone, you will need to visit the Registration Department in person 30 minutes prior to your check-in time.
